Bridges2Success

Funded by the Department of Juvenile Services, the Bridges2Success program provides community-based case management to Prince George’s County youth who are identified by DJS as requiring intensive supervision. The program’s case managers are located at the Leonard Dyer Health Center (Clinton, Maryland). Their work hours are between the hours of 8:30 AM and 5:00 PM.

This program aims to:

• Reduce risk factors
• Enhance protective factors
• Provide screening and linkage to community-based services
• Provide referrals for vocational training and or employment

Client Eligibility

• Prince George’s County Resident
• Referral by the Department of Juvenile Justice
• Department of Juvenile Services Intake Information Form
